The route begins at US 64 near Coal Hill. It runs north across Interstate 40 and AR 352 before turning west. A concurrency begins with AR 103 into Clarksville. The route again meets US 64 before exiting town headed east. AR 164 runs east with AR 123 until Hagarville and continues alone east into Pope County. The route eventually meets AR 7, and the routes concur south to Dover. South of Dover, AR 164 runs southeast to cross AR 124 and terminate at AR 105.
