The Prince Mohammad bin Salman League is the second-highest division Saudi Arabian football league system, after the Saudi Professional League. Each year, the top finishing teams in the First Division are promoted to the Pro League, and the lowest finishing teams are relegated to the Second Division. Al-Wehda are the most recent champions winning their fourth title during the 2017–18 season. Hajer and Al-Wehda are the most successful team with 4 titles each.
The winner and runner-up are promoted to the Professional League directly, and the third-place will qualify for the promotion play-offs against the team which finishes 12th in Professional League. The bottom three clubs are relegated to the Second Division.
1: ^ Najran will play at Prince Sultan bin Abdul Aziz Stadium due to the ongoing war in Yemen.
