KY 385 begins at an intersection with KY 303 southeast of Pilot Oak, within Graves County. It travels to the north and curves to the north-northwest. It has a brief concurrency with KY 94. When KY 385 splits off, it travels in a due north direction and meets its northern terminus, an intersection with KY 2422.
