KY 356 begins at an intersection with U.S. Route 25 (US 25; Cincinnati Road) south-southwest of Stonewall, within the northern part of Scott County. It winds its way in an easterly direction. It crosses over Elk Lick Branch and some railroad tracks of Norfolk Southern Railway. It curves to the southeast and intersects the northwestern terminus of Hinton–Sadieville Road. At this intersection, the highway turns left, to the northeast, and immediately enters Harrison County.

It curves to the south-southeast and crosses over the North Fork Twin Creek. The highway curves to the east-southeast and briefly parallels the South Fork Twin Creek before crossing the creek. It has a brief concurrency with KY 1842. It then intersects the northern terminus of KY 3018 (Mill Creek Road). After crossing over Mill Creek, it enters Cynthiana. It intersects US 27 (Cynthiana Bypass).
