A delayed hemolytic transfusion reaction (DHTR) is a type of transfusion reaction. It is defined as fever and other symptoms/ signs of hemolysis more than 24 hours after transfusion; confirmed by one or more of the following:

This can occur up to four weeks after the transfusion.

Delayed blood transfusion reaction occurs more frequently (1 in 20,569 blood components transfused in the USA in 2011) when compared to acute haemolytic transfusion reaction.

One way this can occur is if a person without a Kidd blood antigen receives a Kidd antigen in a transfusion. Other common blood groups with this reaction are Duffy and Kell.
