The fourth series of the British medical drama television series Casualty commenced airing in the United Kingdom on BBC One on 8 September 1989 and finished on 1 December 1989.
The fourth series of Casualty features a cast of characters working in the emergency department of Holby City Hospital. The series began with 6 roles with star billing. Derek Thompson continued his role as charge nurse Charlie Fairhead, whilst Brenda Fricker played state enrolled nurse Megan Roach. Cathy Shipton and Eddie Nestor starred as staff nurses Lisa "Duffy" Duffin and Cyril James. Geoffrey Leesley portrayed paramedic Keith Cotterill and Susan Franklyn appeared as administrator Valerie Sinclair.
Tam Hoskyns, William Gaminara, Belinda Davidson, Ian Bleasdale, Vivienne McKone and Robson Green joined the cast in episode one as senior house officer Lucy Perry, medical registrar Andrew Bower, student nurse Alex Spencer, paramedic Josh Griffiths, receptionist Julie Stevens and porter Jimmy Powell. Gaminara appeared in four episodes, departing in episode four. Hoskyns, Davidson and McKone departed the show at the conclusion of the series as did Nestor and Franklyn.
